Overview

The objective of this course is to help you understand the importance of the Medical Consenter and their roles and responsibilities in the STAR Health program.

 

To ensure successful health and well-being outcomes for children in DFPS conservatorship.

 

We will also learn about ways to support non-DFPS Medical Consenters.

Sometimes, healthcare can include services that are not covered by Medicaid or STAR Health.

 

If a court or practitioner orders a service that the Medical Consenter is not sure is covered or appropriate, the Medical Consenter should consult with the caseworker prior to consenting.

Medical Consenter

 

The person who decides whether or not to approve medical, dental, vision, and behavioral health care for children in DFPS custody.

 

The person that DFPS or the court appoints to make health care decisions for a child in DFPS conservatorship.

 

Medical Consent

 

A decision about whether or not to agree to a medical test, treatment, procedure, or a prescription medication.

 

Medical Care

 

Medical care for children in DFPS conservatorship is generally covered by STAR Health.

Primary Care Provider (PCP)

 

The Medical Consenter must choose a PCP from the Superior network of providers for each child in STAR Health. If not, Superior will automatically assign one. It is very important to establish a medical home with a PCP who will keep medical records, know what medications the child takes and ensure the child receives the health care they need.

 

The PCP will also make referrals to specialists when needed or required by STAR Health.

 

For help in selecting a PCP or ordering a new STAR Health ID card, call STAR Health Member Services at 1.866.912.6283. Only the Medical Consenter can request a PCP change.

Integrated Medical Home

 

An integrated medical home helps caregivers and/or Medical Consenters locate:

  • Primary Care Providers
  • Specialists
  • Behavioral Health Providers

Each child's PCP will be designated on the STAR Health ID Card. Refer to the STAR Health provider directory for a listing of all PCPs.

 

For changes regarding the PCP, contact STAR Health at 1-866-912-6283.

 

The Role of a Medical Consenter

Includes consenting to the child's medical care:

The Medical Consenter participates in the child's medical appointments either directly or indirectly. This may include medical, behavioral, dental, eye care and surgical treatment. The level of participation for each appointment depends on the nature of the care the child is receiving.

 

The Medical Consenter must attend all appointments when a child is prescribed psychotropic medication. This includes psychotropic medication follow up appointments.

By participating in the child's medical appointments, the Medical Consenter is better able to meet their responsibility of being knowledgeable of the child's medical condition and medical history.

 

The Medical Consenter keeps the caseworker informed of the child's care and of any significant medical conditions.

 

The Role of a Medical Consenter Con't

Medical Consenters must make sure the child receives regular Texas Health Steps medical and dental checkups by a STAR Health provider. In the first 30 days after removal, children attend three health care appointments referred to as 3 in 30. The first is the 3-Day medical exam which screens for healthcare concerns and ensures all treatments and prescriptions are in place.

 

All children entering conservatorship are required to have an initial Texas Health Steps medical checkup within 30 days of coming into care. This initial comprehensive exam is critical to determining a child's medical healthcare needs right away and ensuring the child has appropriate medical care.

 

Medical Consenters must ensure the child has these two medical appointments to receive the health care and services they need.

 

In addition, all children, aged 3-17 years, are required to have a Child Adolescent Needs and Strengths (CANS) assessment within 30 days of coming into care to assess the child's behavioral and emotional care needs and determine what services are most appropriate for the child's individual needs.

 

A STAR Health Member Connections representative will offer assistance scheduling these appointments during the welcome call to Medical Consenters. The call may occur as quickly as 72 hours after a child's enrollment in STAR Health. After the initial Texas Health Steps medical checkup children in foster care must get regular preventive care Texas Health Steps medical checkups according to the Texas Health Steps periodicity schedule. All children in conservatorship, ages must also have annual updates on CANS assessments for current assessments of behavioral healthcare needs

 

Designate Medical Consenter

Now that we know the common terminology, we will designate who will be the Medical Consenter.

 

  • Next, you will document the Medical Consenter(s) information in to IMPACT 2.0 because it will prefill the information..
  • Finally you generate a pre-filled 2085B, Designation of Medical Consenter, gather signatures and disburse document to the appropriate persons.

Information for the Medical Consenters

Important Information

 

Callers to STAR Health Member Services will be asked to verify that they are either the child's caseworker, Medical Consenter, or caregiver by providing their PID or the child's PID.

 

Only the child's Medical Consenter can select or change the Primary Care Provider. Communication between caseworkers, caregivers, and Medical Consenters is pertinent in those cases where the caregiver or caseworker is not the child's Medical Consenter.

 

The caseworker must provide permission to STAR Health in order for the caregiver who is not the child's Medical Consenter to speak to STAR Health when they call.

 

Resources

 

DFPS has a CPS Medical Services Division consisting of Well-Being Specialists, Nurse Consultants and program specialists with subject matter expertise. Most are housed in regional CPS offices. There is also a CPS Medical Director who is a part of the team. Well-Being Specialists and Nurses help caseworkers and Medical Consenters with any STAR Health issues.

 

The Well Being Specialists and Nurse Consultants provide regional support and subject matter expertise throughout the state. Caseworkers can send an email with questions related to STAR Health issues to the DFPS STAR Health 3 in 30 mailbox. A DFPS Medical Services specialist will respond directly to the caseworker.

When the court authorizes DFPS to consent to the child's medical care, the caseworker designates a Medical Consenter and a backup. Then, the Medical Consenter or Medical Consenter backup decides to whether or not to consent to medical care.

Complete and Issue Form 2085B Designation of Medical Consenter

Completing and issuing Form 2085B Designation of Medical Consenter is also an essential part of receiving timely medical care. This form serves as documentation of who the department has designated to consent for medical treatment of the child.

 

For caregivers who are Medical Consenters, having the properly completed Form 2085B is especially important.

  • STAR Health providers use it to verify that the person is indeed the Medical Consenter for the child.
  • It also verifies that the child is in DFPS conservatorship.
  • It assures medical providers and others that coverage in the STAR Health program begins at removal.
  • When completed in IMPACT 2.0, the information for the 2085B alerts the STAR Health program when the Medical Consenter has changed.

You can generate the Form 2085B in IMPACT 2.0 from the Medical Consent detail page or in the Smiley Face Forms link under child placement if you are not able to generate the form in IMPACT 2.0.

 

Please Note: If the court has directly authorized an individual as a Medical Consenter, the court order can suffice as proof of the identity of the Medical Consenter.

Person Detail

Once you have selected the Medical Consenter, that person needs to be added to the person list, unless it is a DFPS staff.

 

To ensure that you are not duplicating person information, do a complete person search in IMPACT 2.0.

Like the placement documentation, the Medical Consenter detail page in IMPACT 2.0 must be completed on the same day of designation or by 7:00 pm the next day.

 

All children must have at least one primary and one back-up Medical Consenter.

Remember to ensure that the address and phone number information is complete and accurate for caregivers as well as Medical Consenters, secondary primary Medical Consenters, and secondary back up Medical Consenters.

 

If address and phone number fields are blank, be sure to add this information.

 

This is an important step because STAR Health uses this contact information to call or to send important documents to caregivers and Medical Consenters.
