Requestor Information

Requestor Information section on form 2277

  • Fill in the Requestor's Information to include the role, a good contact number, mail code (if Court of Continuing Jurisdiction (CCJ) and/or Paternity Registry are being requested, this is where FINDRS will send the returned documents once received from DSHS Vital Statistics), valid email address. The same information would be included for the Caseworker and the Supervisor (if different than the requestor).
  • Provide the County and the Region, the Cause Number (if known), and the IMPACT case number.

** If the cause number is not known, staff must indicate, that no cause number is available.

  • If the requested information is not provided on the form, it could result in the form being returned unprocessed for updates.
