You can generate the Form 2085B in IMPACT 2.0 from the Medical Consent detail page or in the Smiley Face Forms link under child placement if you are not able to generate the form in IMPACT 2.0.
Please Note: If the court has directly authorized an individual as a Medical Consenter, the court order can suffice as proof of the identity of the Medical Consenter.
